Appointment request form

The form prepares a message in your browser using the details you enter. Reviewing the request does not send it to the clinic or reserve an appointment. Opening WhatsApp includes your details in a prefilled message link shared with WhatsApp. You must tap Send in WhatsApp to send the message to our team.

Keep your first message simple

Please use the request for contact details, scheduling preferences, and general questions. Do not include medical records, identity documents, photos, or other sensitive health information. Discuss any clinical information directly with the clinic through an appropriate channel.

Messaging services

WhatsApp and Messenger are external services with their own information practices. See the WhatsApp privacy policy and Meta privacy policy. An appointment is confirmed only when our team confirms it with you.

Website measurement

The website uses Vercel Analytics and Speed Insights, and supports Google Analytics and Meta Pixel when configured. These tools measure visits, performance, and interactions such as contact-link clicks. The appointment form does not include your name, phone number, or free-text message in its analytics events.
